How do I determine what version I have?

There are 4 ways to determine your version.

1) Look on your CD (if you have one) and find the Disk #. The first 2 digits of the Disk # is your version. Example: 6103CD0001404WLP would be version 6.1.
2)

When you start the program a "splash" screen comes up. The version is shown one it.
3)

When the program is open to the Data Entry window, click on the Help menu and select About Wellness Profile ...
4)
 If you don't have your CD, or never had a CD, and the program is no longer installed on your computer, then complete the form below and I will email you your version.
